Chances of Transferring Back to my Home School's IM program as PGY-2

My fiance is in a different city and I am in the middle of my intern year in another city. What do you think are the chances of transferring into an IM program in her city (AND my home school's hospital)? Right now I'm at a community-based hospital...from avg med school, did avg-above avg in med school.
