My #1 choice for IM has both Primary Care and Categorical tracks, and they allow applying to both. However, the ranking is separate. At the interview, I gave the impression that I was interested in categorical, but if I've now changed my mind and want to apply to the PC track as #1, would it be worth to contact the Primary Care track director to make her aware of this? Or I would I appear fickle?

I would send an email. I don't think it makes you look fickle and I don't think you have much to lose.

You can always do primary care from a categorical spot. You can also apply for fellowship from a primary care spot, but it is more frowned upon. Categorical gives you more career flexibility, and you might end up not liking primary care...
