Is anyone struggling with responding to secondary questions about personal difficulties or ethical dilemmas or whatever without sounding like you're making a big deal out of something that really isn't a big deal?

Yes, but we've got to answer the questions anyway. Not everyone who gets into med school has had a tragic life. Just write about something that was a personal struggle. I think it's more important to schools that we show that we can learn from struggles and move on, even if they aren't a huge deal. There's really no reason to overdramatize and make it seem like your life was ruined over something small. Just say how it affected you and what you learned.
 
I think that the point of these questions is not to choose the applicants with the saddest or most outrageous stories but to see how you approach a personal difficulty or ethical problem and how you resolved the issue. What are your values and how did your values influence the decision you made?
