Umm i dunno bout other colleges but i go to KIMS in bangalore... and I know that you can do your internship of 1yr back in the US provided that you get all the postings you want for the duration you want.... now the problem of arranging that is totally yours as KIMS has no affiliation with american med schools or such so it helps if daddy is a doctor and has a lot of doctor friends... but yeah the option is there.... the other problem is that there prolly wont be a community medicine posting in the US so you will have to do that 3 month posting in KIMS. I know that ramaiah in bangalore operates like this too... I'm sure there are more colleges that you can do this with but I dont wanna just throw out names without being sure... For KIMS and Ramaiah I am sure... They are both in Bangalore... hope this helps
