code for facet arthropathy

what code does everyone use for facet arthropathy. Does this code need to be used when pre-certing facet injections?

The odd thing is, when I am stuck with a T12, L1 compression Fx and there is facet mediated pain, I have to bill 4 different codes to cover T11, T12, L1, L2 MBB's. Feels dirty, but have only needed to do this twice in 3 years.
