Combined residency of FM and Preventive medicine

Hey, anyone knows about the combined residency of FM and preventive medicine? I am looking particular in California. So far I only found Loma Linda has such program. Anyone knows other school who does that?? I don't mind going to East coast, but please I need to stay at the coastal area or some big/college town.
Besides that, anyone know what are the stats for those residencies? Step 1 score and other eligibility that can boost one's application?
