Comminuted Mandible Fractures

How do you treat severely comminuted mandible fractures in your program? Closed reduction, Joe Hall Morris, ORIF? I'm talking about shredded mandibles, like someone s/p GSW to mandible. Currently we are trying to get closure of soft tissue lacs and close reduce the patient, then reconstruct prn.

The ENT department did a suicide attempt today by opening the fracture and putting approx 453,729 plates on all the pieces of bone. I'm not sure that's such a great way to go.
 
Here in Dallas they get treated by Periodontists, so I couldn't tell ya.
 
In Louisiana they use Santeria to heal those sorts of things.

DIPSTICK....in La we use an Ex-fix....usually the Synthes radial forearm ex-fix....works great for mandibles.....easy as hell to put on.....like a tinker-toy to assemble. We put on like 15 in over a 3 month period in Charity....we still use the same think in BR.

we just did one...we used an ex fix. Synthes has a new ex fix for the mandible (so you don't have to use the radial forearm one). Its nice because you can adjust the vectors in any plane that you want and they have pre bent rods that can go from ramus to ramus.
 
Yeah dude....i forgot about the new mandible one...I think that's what we used a month ago. Regardless.....they're so much easier to use than a Joe Hall Morris. Actually, I've never used or made one. We've always used the Synthes. Much easier to take of than Joe Hall too.

It is so funny this post is out there. I went to assist in the OR today and a sophmore dental student was in there. The residents were placing an ex-fix and the sophmore asked me...what is it like? So I told him...it's kind of like attaching an erector set to someone's face. So he says...."I don't get it, what will it look like?" So I told him....this guy's will look like a double rung, gold-plated, highly decorative towel rack hanging from the outside of his mandible. The poor kid was in shock.....and when they pulled out the bolt cutters I thought he was gonna faint. 😀

out here, we hang a complimentary christmas ornament from the ex-fix while the patient's in recovery.

See...we discussed that, but we figured we'd save that for the holidays. Basically this guy was from the sticks....all he needed was a bug shield and a Ford emblem and he would fit in perfectly when he returns to the panhandle

You just dipped your own hands in a scrub sink for the first time five minutes ago and you're already playing tricks? Shame on you..

Nope...even worse...I'm still a worthless dental student w/o a diploma....I've only dipped my hands in to assist....haven't even made it to orientation yet. It was just entertaining thinking back to when I was a sophmore dental student and all the seniors used give me crap 😉
