Confused - 2 Questions about LOR situation

I have 2 quick questions and appreciate if anybody can help.

1. One of my letter writers forgot to sign the LOR, but letter has already been sent to schools. Actually, it was one school that actually told me that it wasn't signed. What do I do?

2. This one is a little strange...my school has a letter packet system and I only partially used it. I also have letters sent as individuals. When I'm answering secondary questions about type of LORs I'm submitting, I find it difficult to answer which type I'm sending...
