confused about washington's requirements please help!

Is anyone familiar with Washington state's requirement for recent grads/score transfers? According to the state board of pharmacy's website I'm required to submit preceptor eval and intern site eval. Does this mean that I need this form filled out for every APPE? Sorry I'm coming from a different state and we use different forms at my school. Plus I tried calling the state board but the phone line is always busy. I really appreciate any info. Thanks!
