Contemporary Problems in Medicine

Greetings All,
What are the some of the problems facing physicians at present? I've been told of several by some physicians, and would like gain a better understanding of such issues (such as physician autonomy, healthcare, and insurance to name a few) From my interactions with physicians so far, responses have been quite interesting. Can anyone elaborate on these issues?
