when a pharmacist wishes to clarify a controlled II, III or IV script, the following apply:

Without a consult, a pharmacist can add or change address.
With consult, a pharmacist can change dosage form, strength, quantity, and directions.
A pharmacist can never change a patient name, drug, or physician signature.

A lot has happened in the eight years since that post. Is this still up-to-date?

I think it depends on state. In TX, for controlled II, you can’t change name of prescriber, patient, and drug, as well as the date, everything else you can call physician and modify it on the original script with initial for both physician and pharmacist. For controlled III-V I believe you can change evething as long as you call the presciber, normally you just get a new prescription instead of change the old one.
