I know that Occupational therapy assistants can't get certified in hand therapy as its only for actual OT's but can COTA's work in any type of hand/ upper extremity rehab?

Yes they can! The OT I shadowed in the hand ortho practice mentioned that she used to work with COTAs in her last job.

I also shadowed an OTA that recently switched from working at a hand clinic to pediatrics. She mentioned that she would work with the patients using the hand exercises that the OT had assigned.
