Credentials necessary to gain admission into MD/joint degree programs

After recovering from yesterday's MCAT fun, I have finally started my secondaries. I was wondering what type of credentials does one need to gain admission to MD/joint programs. Do those spots usually go to applicants that are the most brilliant? If you don't get into the joint program will that hurt your chances of gaining admission into the medical school? Also, how to do current medical schools students regard students that are pursuing dual degrees?
