CRNAs practicing independently on non-opt out states

How does this work? Specifically for Medicare patients. Do they just not do medicare cases? What about medicaid? Same rules? Does the surgeon take supervisory responsibility when it's a government insured case?

I know supervision is not needed anywhere for CRNAs in non Medicare cases.

Asking because I have seen groups in my home state of TX where CRNAs are practicing independently. And Texas in NOT an opt-out state.

If the surgeon "supervises" them then they can do whatever they please.

Surgeon hires own CRNA. Surgeon signs CRNAs records. Much like some GI docs.
