deferment letter to pharmacy school

I am about to prepare a deferment letter to one pharmacy school for my family personal problem. Could you please help me how to make it a good one & what else I should include along with the letter?

First be sure that the school even offers deferments, then make sure that your reason is one that qualifies if they do. For your letter be specific about why you need to get a deferment. If possible send some sort of doctumentation (ie: medical condition) with the letter.
