One of my family friends who I’ve known since I was a small child graciously hired me as an MA in his private practice.

Is it frowned upon to get a letter or recommendation from him? It definitely would be a professional letter based on my time working for him but he would be stating that he’s know me for ~19 years. Would this raise some eyebrows?

He should limit to professional contact period not for diaper days.

Yeah I'm no AO but I'd probably weigh the letter less if it started with "This kid is my best friend's son and I've known him for 20 years..."

I don't think it will hurt you. I had a family friend who I shadowed write me a letter and had no problems. I think he disclosed that he knew me before I shadowed him and it wasn't an issue!

I would not read this as an unbiased letter from an employer but a very biased letter from a family friend/neighbor.

It's not quite as bad as a (doctor) Mom writing a letter for her darling son, using her unmarried surname to hide the relationship.
It is along the same lines, though.
