Is it worth going to PENN instead of Buffalo with instate tuition if the difference is $50,000 for 4 years (interest not included)

Didn't OP already ask this question? I feel like the only reason to re-ask a question like this is if you didn't get the answer you were looking for the first time around.

OP, we can tell you our opinions, but at the end of the day this is your decision and you should choose which option you think you're more likely to be able to live with.

Not if you intend on being a GP. If you are open to/plan on specializing, there is a slight, almost negligible advantage in attending schools like UPenn when you're competing for residency spots. And even then that felt advantage is limited to only wooing a handful of residency directors. Another thing worth considering is if your ego is attached to the name of your school (different intangible values). Since we're all self-employed, a specialist from UPenn or Buffalo will have equal opportunities in making the same amount (same tangible values). Even if you are employed, your employer will not care about your alma mater. Referred patients won't care and your referring GP's won't care. Only you will care, that is, if you even care about the name of your school.
