unless they talk to each other do PD's have any way of knowing where else you applied to via ERAS? whether it be program OR location/hospital?

No...they should not and it is illegal for them to talk to other PD's or to ask you where else you have applied. That said, the rules are still bent because the rule is difficult to enforce. Some PD's do talk to each other if they are good friends but for the most part, the majority of PD's will not know where else you've applied nor is it their business to know.
 
If you do something incredibly stupid along the interview trail, you could very well find out how many PDs do talk.

If they are friends, some might but PDs know that you are applying to multiple places. That being said, I would not tell more than one program that they are your first choice. The most diplomatic way to handle it is that you plan to rank them very highly. (if they ask how highly, reply that you have not finished your interviews and it would not be fair for you to answer until you have).

Unlikely, however it's a small world, and a smaller profession. When you break that further into specialties, it gets smaller still. Assume everybody knows everybody.
