Just a thought.

Is it ever appropriate to let a school know that they are your first choice before any decision is made on your application?

For example, if appropriate in a "Why do you want to come to <INSERT YOUR FAVORITE MEDICAL SCHOOL HERE 😍 >?" secondary question, can and how do you let the school know their superiority in your preferences?

Is it even worth expressing this opinion before you hear some sort of positive reply/feedback from them?

There's nothing wrong with it. It won't hurt your application but it probably won't help either. The only school I praised effulsively during the interview waitlisted me right up until classes started. Personally I think it makes you sound desperate when your too complimentary. Just let them know what specific parts of their program you like the most. That's what they want to hear anyway.

In your secondary, tell them why they are your first choice. For many of the schools, they'll ask you in the interview. Schools want to know they're your first choice, but perhaps the secondary is too early. Just my opinion, though.

Yes definitely. And be sure to have compelling arguments for WHY they are your first choice too.

The first person I ever interviewed for medical school told me that we were his first choice during the interview. When I received a thank-you note afterwards I was again informed that we were this individuals' first choice.

We accepted this person (not because of the declarations, mind you), who then proceeded to matriculate at another school.

I hope this anecdote has been informative.

In my opinion, I'd say writing that a school is a top choice in the secondary is too early. I would instead focus on things you really like about the school and why you'd be a good fit on the secondary.
