does anybody mix calcium into their protamine for coming off CPB?

Not sure what the compatability is.

And that saves how many seconds?
 
Not to save time but to give an inotropic boost with the proti and to do so in a hemodynamically smooth way by having it mixed in.

Protacal. One institution I worked at long ago used to do it. Gave it via soluset.
 
We do it routinely. Then again, we put Swans in virtually every heart patient, unless contraindicated.
 
Protamine goes through the PIV where I train. I try not to give concentrated CaCl2 peripherally, hence no mixing for me.
 
Buretrol with

1 g calcium

Protamine

Saline to bring to 50ish cc

Run wide open through PIV

Flush with additional 25-50 cc

Works great. I can't even tell that the prot is going in unless I am paying very close attention to the PA pressures which occasionally bump slightly.

NIce tip. ...don't currently do cardiac, but will remember it.
