Doing internship without a residency and then applying

If you get accepted into an internship but not a residency program, can you apply for a residency during your internship?

When you apply as a medical student the residency is the year after your internship so if you apply during internship will you have to wait a year until starting residency?
Are you limited to PGY2 spots from people who leave the program in this case?

If you only match into an internship is it better to do a year of research and reapply? Can you get LORs during internship and get a stronger application?

Yes you can apply to match into another PGY1 year or an advanced residency during intern year. Yes you would have to wait a year if you were already an intern and matched into an advanced path. Advanced programs start in PGY2, so those are all available to folks in or accepted to PGY1 years even without anyone leaving. In most cases if someone leaves that gets filled outside of the match with someone who already has an intern year under their belt. Once you match you are contractually obligated so you can't just do research. You have to decide to do this before you enter the match.
