1) Ophthalmology (SF Match), Urology (Urology Match), Military Match
2) Everything else
3) Maybe plastic surgery, but I think all the integrated spots are through NRMP now.
Edit: This is only residency match. Fellowships are a whole different ballgame, their deadlines are all over the place if they even use match, many don't.

If you do ophthalmology, which includes a transitional year, do you have to register for both the early and regular matches?
