During MMI can you ask questions to the evaluator?

I guess you would have to show your thought process by saying things out loud like "Before I decide on _____, I would need more information"

But can I ask "does this patient have any other health problem?" or "does the patient __X? or Y or Z?"

or do I just say "first, I would check if the patient has any other health problem or X or Y or Z. "

what's the general rule?

Yeah sure, but it's just entirely dependent on the situation presented. Some MMI's are more back and forth, some not, you'll have to gauge it yourself and see how it flows.

I think a good practice is turn your questions into statements. Like, “I need to first find out about ______. If I discover that ________ then I will do x y z. If I find that _____ I will do abc.” Part of the MMi is seeing if you can reason through possible situations.
