Would it make a difference to your (or anyone else's) decision if i mention that it is a 5cm ethmoidal sinus primary eroding the medial orbital wall and focally involving the orbital fat? Margins are clear through.

Appreciate all inputs. Most of the literature are/are based on small case series, and I just want a feel for what most would do.
 
worry about disease coming back at the primary site, where the margins are likely to be very close. Even adjuvant radiation in melanoma never had a survival benefit. Pt may be eligible for adjuvant ipi or nivo (recent trial).
 
That should say adjuvant RT "in node-positive melanoma" never had OS survival benefit. In node (-) that would certainly hold; additionally elective node RT in this regions would carry high morbidity.
