Endocrinology fellowship and Practice

I am in the starting process of applying to 2007 Endocrinology fellowship, but have a few questions. Is there a ranking of the top endocrinology fellowships? How typical is it once you have completed the fellow for a person to go on to practice in the same city? Also, can anyone shed some light on where the best places to practice endocrinology are. I am mainly focused on Diabetes. I have looked at the AMA site and I know the problem states, but I would hope that this would change by the time I am done with fellowship. I have also looked at the US news report on the top 50 hospital for Hormonal disorders, and believe that that list is not a good source since some of those listed to not offer fellowship.
