Community of Hope (COH) is a nonprofit organization that supports low-income and homeless individuals, children and families in the District of Columbia by providing housing, healthcare, and supportive services. Position Summary:

The Family Medicine Physician will provide primary care to a diverse population of pediatric, adult, and prenatal patients. This position reports to the Lead Family Medicine Physician.

Minimum Qualifications:

  • Medical Degree (MD/ DO) Required
  • Board Certified preferred
  • Licensure in the District of Columbia
  • CDS and DEA required
  • specialization in family medicine and the ability to care for adult, pediatric, and prenatal patients required
  • Experience using an EMR preferred
  • Bilingual in Spanish or Amharic a plus.

Highlighted Duties and Responsibilities:

  • This is an outpatient only position, with shared telephone on-call responsibilities.
  • Our clinics benefit from the full implementation of eClinicalWorks EMR and co-localization of dental and behavioral health services.
  • The successful applicant will have the desire to work with medically fragile patients with complex social needs, preferably with experience or training in underserved medicine.
  • Provides direct medical care to patients, following approved guidelines.
  • Refers patients to specialists as needed.
  • Refers patients for case management as needed.
  • Communicates with other providers on issues of patient care.
  • Provides care efficiently, including maintaining flow.
  • Signs off on all laboratory results, diagnostic imaging results, and referral reports in a timely fashion; sharing this information with patients as appropriate.
  • Completes all e-refill requests in a timely fashion.
