At the undergraduate level, it seems that private schools give tremendous help to students below a certain income level.

Do private schools continue to do this for their medical students that are in need? (Extreme example: A med student with an EFC of 0)

Only if the school has the funding/endowment to support it. While not entirely correlated, the higher tier the school, the more need-based and merit-based scholarships they can offer.
