I was wondering if anyone knows whether step 3 is required for foreign nationals who are us medical graduates and applying for h1 visas

Step 3 is required for all H visas, regardless of where you are doing your medical school. So, yes, you need Step 3 for an H1-b.

However:

If you are a non-US citizen in a US medical school, you should have an F student visa. IN that case, the F visa can be extended for one year for OPT training (which internship clearly is). This would allow you to match, continue your training on your F Visa / OPT / EAD, and then write Step 3 in your internship and get an H1-b.
