General vs ENT?

Here's a question: For the first five years of training, I would probalby enjoy the general surg residency more than the ENT residency. But, I would rather due a Head and Neck Onc fellowship over the fellowships following the general surgical residency. What do I pick given that a lot of things can happen between now and when my training is done? Thanks.
