Gouty facet disease

    Anyone seen gouty facet disease, and if so, what have the MRI findings been and did you get good results from facet inj?

    Yes, a colleague of mine, a pain doc himself diagnosed himself with facet pain. Got another pain doc to perform MBB i believe couple of times with no relief. He remembered he had a h/o gout in the past in more peripheral joints and so adjusted his diet and started taking his allopurinol again. Back pain went away. I don't know if he did MRI

    thats not EXACTLY an iron-clad diagnosis.....

    if they got some crystals from an aspirate, then we are talkin.

    As a follow up, my patient did great. He was first evaluated in a wheelchair with his facet pain, two facet injections into the ONE facet joint and he was completely pain free. Now walking without a gait aid and pain free. His initial presentation was for an intra-articular hip injection...without groin pain, that one was doomed to fail.
