The dentist I work with now did her's at Miami Valley Hospital in Dayton.
(http://www.miamivalleyhospital.com/dental-desc.htm) - she really liked it and spoke highly of the program. She said the centers are operated like private group practices, with annual salary of $43,750.

OSU has one with 2 tracks; hospital or clinical. They have 8 spots each year, don't know much else about it.

Forum Health Dental Services in Youngstown has a program too.
