I have some private loans I wanted to get paid off prior to working once residency is done. Is it possible to negotiate that the employer pay this off lump-sum (in lieu of a bonus for instance) for say 2 or an extra 3rd year in the contract? I think from a tax-perspective it might be better too.

And how much do you think you could negotiate max? Thank you.

It's still taxable compensation beyond $5250 a year. Many larger corporations are doing this now. Fidelity jumped into the fray in late 2017.
