what is the mechanism behind gynecomastia and hirsutism (in female) in liver disease?
what are the role of SHBG, aromatase, 5a-reductase in these?

Gynecomastia is due to increased circulating oestrogen's caused by reduced liver metabolism and increased peripheral conversion of testosterone. Palmar erythema and spider angiomata are also because of the increased estrogen. For hirsutism as far as I know liver disease decreases SHBG so there's more free testosterone leading to hirsutism in women.

I think gynecomastia in women would be called puberty.
