Hey guys the way things are going right now it appears that more and more programs are making it harder or impossible to sponsor H1b visa. The annual cap has dwindled from over 160,000 to 65,000 starting in October. And many residency programs are being pressured to extend the 2-year J1 home visit to 5 years. These changes are--no doubt-- as a result of 9/11... and who can blame America. However, the problem is there are very hard-working well-meaning students that are being affected. Basically if you have an F1 or F2 status or are an IMG stay away from J1 because it is a trap. The good news is that there are good institutions out there that will continue to sponsor H1b visa and I urge us to help each other out in identifying those institutions. So I IMPLORE YOU TO HELP ME MAKE A COMPREHENSIVE LIST. I am aware of 3 level 1 programs and I level 5 for general surgery and I will begin this list; I will add more as I do more research but I invite you to chime in.

H1b visas are only given to students who have taken Step 1-3?

Yes, to qualify for an H1b visa, one must have had passed Steps 1-3.

From what I've gathered, Baylor, Harvard, and Cornell sponsor H1b. I also noticed that California schools tend NOT to sponsor any visas at all.
 
So for a FMG (Canadian) who wishes to do residency in the States, only the J1 visa is available? Is a H1b the same as a green card? Is it possible to obtain a green card while doing your residency (or during school in Europe)?

University employees (research and physicians) are exempted from the H1B cap. So that is not the factor behind these issues.

^That's true. The main problem with the h1b visa is the paperwork the employer has to attend to. They have an easier time with j1s because the ECFMG takes care of that.
