I've heard great things about it from some ppl while others seem to have no idea what it is. I searched around here on sdn but the most recent thread about it, is from 2006... http://forums.studentdoctor.net/showthread.php?t=254085

Has anybody used it in their step 1 prep recently? And if so, how helpful was it for you on the actual exam?

he's one of valuemd's step 1 gurus (much like sdn has taus :)): http://www.valuemd.com/usmle-step-1-forum/154397-tommys-high-yield-post.html

tommy ended up creating like around 3000 questions or something like that to help ppl understand the concepts... back in the day a lot of ppl did them (my brother did too when he was in school) but the popularity seemed to have faded over the past few years (whether due to fact that they're not good enough for the exam these days or maybe ppl simply forgot that these Q's exist since goljan HY lists came into the picture). the sheer volume of questions seems to be a bit too much when there's obviously better ones out there like UW but it seems like it would be good for IMG's and ppl like that who have a little bit more extra time on their hands.

example of typical tommy HY questions: http://usmle.valuemd.com/tommykusmlestep1.htm

I've heard a lot of great things about Tommy, so I printed it out (all 598 pages). But I wish it were organized by subject or systems. Does anyone know where to find them organized this way?

i wish that it was organized like that too! it would make studying it so much more easier... unfortunately i only have the word file where all the questions are randomized. is that what you have too?
