Just out curiosity what are people using as indications for treating elective nodes for a high risk squamous cell carcinoma of the head and neck. I have previously used the following paper to talk about risks of nodal involvement with patients:

Evaluation of American Joint Committee on Cancer, International Union Against Cancer, and Brigham and Women's Hospital Tumor Staging for Cutaneous Squamous Cell Carcinoma

curious as to what others are doing. Treating only first echelon nodes? Easy ways to get IMRT covered?

Limited experience here, but maybe you'd find these papers helpful.

"A new evidence-based risk stratification system for cutaneous squamous cell carcinoma into low, intermediate, and high risk groups with implications for management." PMID: 28917382, builds on the Brigham study you cited.

"Predictors of outcome in patients with metastatic cutaneous head and neck squamous cell carcinoma involving cervical lymph nodes: Improved survival with the addition of adjuvant radiotherapy." PMID 22109745, some more detail on locations of nodes involved in node positive patients.
