History of hospitalization for mental illness and California licensing

Hi,

I want to practice in California and read somewhere that the licensing application asks if you have had any hospitalizations in the past 10 years for mental health issues. Can anyone who has been through the California licensing process confirm this? If so, is there a place to explain that issues have been resolved and it would not interfere with medicine? Thanks.
