How do i register to take the DAT's

I feel like a complete *****. I took the DAT's last year and I'm taking them again, but I can't seem to remember how i registered for the test. There was some kind of booklet that I filled out; however, it doesn't seem to be available. Has everything been moved online? Thanks.
