How do programs know where you are originally from?

I was born and raised in Chicago, but went to med school on the east coast. I want to go back to the Midwest for residency. I would like programs to know my geographical ties.

So, how will programs know where you are from before any interviews? Do I put it in the personal statement? Or will the "permanent address" area on ERAS be enough to clue them in (do they even look at that and care?)?

Your place of birth is on ERAS, as is your undergrad. Most people I interviewed at least mentioned one or the other of those.
