I'm talking about a program like rosy f or Tulane...

If you essentially apply to med schools right when you matriculate in the smp, how do schools have anything to go off of?

You'll have to update them with your grades in January and hope for the best.

Truthfully, SMPs are best to apply to the home institution if you're applying for entrance next year. A gap year (called a glide year) would be best to do since it isn't impressive to just be in an SMP. The home institution knows about its own SMP and will look at your fall grades and possibly spring if they decide to wait list you so when doing an SMP, it's always best to aim for the home institution you're doing it in.
