CO Medicaid is stating that 64636(lumbar rf add level) is an uncommon code and creating alot of headaches for me to get paid. Does anyone have info on how many times Medicare paid for the code last year? I know the number of ESIs performed each year are mentioned by some so the numbers have to out there.
