How to address thank you letter

OK, so I'm a little confused about how to address my thank you letters. Should it be:

XYZ School of Medicine Office of Admissions
C/O Dr. Knowit All
123 University Ave
BigCity, AL 90210

or

Dr. Knowit All
123 University Ave
BigCity, AL 90210

or

Dr. Knowit All
XYZ School of Medicine Office of Admissions
123 University Ave
BigCity, AL 90210

I'd go with:

Dr. Knowit All
co/XYZ SoM Office of Admissions

This is what I've done on mine.

That's what I've been doing.

The "care of" (c/o) means you're sending it to a third party to pass on to the individual in the first line. I.e. in the above option it's care of the admissions office, which will forward it to Dr. Knowit All. Don't use your first option, it's backwards.
