How to improve?

Only a few months left in M1. I've been getting about half H and half HP for the courses. Really frustrated from time to time, because for the HP's I've got, I missed H by 1-2%. It seems I just tend to make really stupid mistakes in exams, mis-reading either a question or a answer choice and stuff like those kept me from getting H in those critical courses (I heard from the upperclass that if not getting H in these courses, I am putting myself in the upper half of the class, but not the top 20%).

I am aiming for competitive specialties such as Rad or RadOnc, and our school ranks us (either by 10% or quantile I think) in the Dean's letter. What can I do to improve?
