Could I walk into any hospital or clinic in my state ( or the U.S.) and start treating patients?

Absolutely not.

You're conflating two different things and ignoring a 3rd super critical thing.

One - Licensure is different than board certification.

Licenses are provided by state medical boards and allow you to work in that state.

Board Certification is an representation of medical knowledge run by national specialty boards. It has no legal status.

To actually treat patients you must be credentialed by the hospital you are at - "have privileges" in the colloquial sense. Credentialing limits you to specific things based on your actual training. I'm credentialed in Pediatric Critical Care at my children's hospital which allows me to do certain things that are different than someone credentialed in Pediatric GI or Pediatric ENT. The Pediatric Surgeons aren't allowed to do the same set of surgeries as the Pediatric Orthopedic Surgeons.
