At what point during the Internal Med residency does one apply for fellowships?

Early PGY-2-- about aug-nov of your PGY-2 year would be the optimal time.

If you want to go right into fellowship after residency, then it's around beginning to mid 2nd year. GI is pretty early, whereas Cards is more in the early fall/fall. The others fall somewhere between the two. Cards, GI, Pulm have matches and I'm not sure about renal, ID, etc.

A lot of people do a hospitalist year after residency to make $$ (175-250k are some of the offers people get from my residency program) and then start fellowship, so they apply during 3rd year.

$250,000 for a hospitalist?? That seems really high. Were there any special circumstances?

Q5 call at a hospital where when you are in house, you are the CCU and the MICU doctor (open units) in addition to the admitting doctor. Also, I train at a top 5 IM program, and the people coming out of here tend to get pretty impressive offers.
