so when applying to fellowships does a graduate from an american medical school have an advantage for an international medical graduate even if both attended the same residency program... and if so how much of an advantage does the american medical graduate have.

Infinetely more - because the #1 measure of reputation of a residency program is its resident's website so imagine what's more impressive:

i think the whole FMG thing follows you thru all your training levels, regardless where you end up for residency

Classic! This is the funniest thing I've read in days. Seriously, I don't think being an IMG matters if you're both from the same residency. Both residents would have the same training.

A lot of IMGs have the same problem applying to fellowship that they had when applying to residency--they need a Visa. It may be harder to get a spot if the program doesn't really want to sponser you for a Visa.
