immunizations before entering med school

Can anyone enlighten me on the specifics. I'm pretty sure that I'm missing some of them.

UCSF requires immunizations or positive titers (sp?) for measles, mumps, rubella, chickenpox, hepatitis B (2 out of 3 shots), and the tuberculosis skin test.
 
Our school, WVU, required us to all at least start our immunization process before we started our first year. These included the ones mentioned above, MMR, HepB, PPD, and Tetanus. We had to show proof of polio vaccine. Afterwards, we had to have titers done showing immunity to Mumps, Rubella, Rubeola, HepB, and Varicella. These had to be done before we were allowed any patient contact.
