Meh. It depends on multiple factors: your field of practice, your personal goals, your ability to get in, the needs of the community you practice, the national political environment that drives reimbursement rates, and your love of the work you do.

It depends. In some fields, fellowship is the expected norm and not having one means you are at a severe disadvantage when seeking employment. In other fields, a fellowship can allow you to practice credibly in a specific subspecialty, ie to make the move from generalist to specialist. And it may be of import in certain academic paths. So yeah, in many cases it is important. If you want to be a generalist in primary care and just hang up a shingle, it wouldn't matter a lick.
