Internship Year Confusion Before Residency!!

When applying to residency in dermatology, is the one year internship essential and a must, and if so, is it included in the 3 year res plan. Also I am thinking about the combined residency program of derm/Im...it is 5 yrs....does that include the internship year, or is there no internship year.
This small internship deal has got moe thoroughly confused. Help!?
A side queston: What kind of research opportunities exist for 1st year meds and on, if they know derm/internal med is what they want to do? Thanks friends.
 
1) The intern year IS essential, total 4 years with the 3 years of derm.

2) Derm/IM is 5 years, I think it's 2 IM/3 Derm, but very few programs are doing this.

3) If you are in IM and want to try to get into derm, you can a) jump ship after intern year and do a derm research fellowship (check listings at p204.ezboard.com/bdermatology), which risky if you already are in categorical IM, but par for the course if you're just doing prelim, or b) if you're categorical, you could always stay in IM and keep applying to derm.
