I am in a top 20 university. I have cGPA 3.88, sGPA 3.87, MCAT 517.

Lots of research experience, with one first authored paper and one co-authored paper.

medical clinical volunteering, teaching and shadowing.

for my stats, is it worth to apply Duke and Washington University in St. Louis.

Buy the MSAR.
Filter for the 57 schools that consider internationals.
Eliminate the mission based schools.
Apply to the ones that matriculated at least 5.
I am not international student. I am a US citizen.

You are a good candidate for most of the schools on your list.
You could consider swapping the lower yield ones (VA, Mayo) for Miami, Hofstra, USC, NW...
